Job Summary:

We are seeking a highly skilled Care Manager for Long-Term Services and Supports to join our team at Centene Corporation. As a key member of our care management team, you will play a critical role in assessing and coordinating medical and supporting services for our complex populations.In this position, you will be responsible for developing and implementing personalized care plans that address the unique needs of our members. You will work closely with healthcare providers, community resources, and family members to ensure that our members receive high-quality, cost-effective care.Key Responsibilities:

• Assess and coordinate medical and supporting services for complex populations
• Develop and implement personalized care plans that address member needs
• Collaborate with healthcare providers, community resources, and family members to ensure high-quality care
• Monitor member status and make adjustments to care plans as needed
• Collaborate with leadership team to improve and enhance quality of care and service deliveryRequirements:
  • Graduate from an Accredited School of Nursing or a Bachelor's degree and 4-6 years of related experience
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure requiredWhat We Offer:
